When you have the Sûreté du Québec ("SQ") verify the criminal record of your school bus drivers, you must use the procedure established by the Fédération, which is found in the Legal Info of the member zone.

According to this procedure, the carrier must send three (3) documents to the SQ, namely:

1. The Statement of Criminal Record completed and signed by the employee or candidate for hire;
2. The consent form completed and signed by the employee or the hiring candidate;
3. The Document requested by the Security Clearance Division of the SQ (Excel spreadsheet) to be completed by the carrier.

This third document now contains a space to include the customer account number. This is a four (4) digit number that you received when you set up your account with the SQ.

If you are unable to find your account number, you must contact the divhabilitationsecuritaire@surete.qc.ca by providing the name of the carrier and the address of the latter or by phone at 1-844-290-2223 .

Please note that your FTA membership number is not your customer account number at the Sûreté du Québec.
